Your First Dental Visit
From the very first phone call, we want our new patients to feel excited about what’s to come and confident that they’re placing their teeth in the right hands. At your initial appointment, we’ll start with a dedicated cleaning that is carried out by our friendly hygienist, followed by a one-on-one comprehensive examination with Dr. Lamond. He’ll take the time to listen to your concerns, hear about your goals, and help you explore personalized treatment recommendations that are a good fit. Finally, our front desk will assist you with financial options and scheduling any future appointments.
New Patient Forms
Before your first appointment , we ask that you download and print the new patient forms, located in the link below. If you are unable to print these pages, just call us. We’ll be happy to email or fax these forms directly to you. Bring the completed paperwork with you to the first appointment to expedite your service.

Dental Insurance Welcome
For patients with insurance, we accept all PPO dental plans and our team will be happy to file claims on your behalf in order to help you maximize your deserved benefits through reimbursement – you won’t have to lift a finger! Before any treatment begins, we will also provide comprehensive price estimates for the sake of transparency, including what we expect your provider to cover. This way, you can approach smile-inducing care with financial confidence. We are in network with Cigna, Delta Dental PPO and United Healthcare.
